How to Access the Memory Clinic

Talk to Your Doctor
Your Apple Hills physician is your first point of contact. If you or a family member are experiencing memory concerns, book an appointment to discuss your symptoms and history.
Referral & Assessment
If appropriate, your doctor will refer you to the Memory Clinic program through the Summerville Family Health Team, where a coordinated assessment can be arranged by the care team.
