What This Form Is For

This form is for non-urgent, general inquiries only. Messages are monitored between 9am – 4pm, Monday through Friday.

  • For appointment bookings please use: patient.medeohealth.com/booking/apple-hills-medical-associates
  • For prescription refills please contact your pharmacy to fax a refill request to your Doctor.
  • If you require IMMEDIATE assistance contact our office by phone — a response to your message can take between 24–48 business hours. URGENT requests are not to be sent through email.
  • If you DO NOT receive a response after 24 hours please contact our office by phone.
  • Please do not send multiple messages for the same issue.

Important — Emails for the following will not be responded to — you must contact our office by phone for the following.

  • Prescription New or Refills
  • Test Results
  • Referral Status Updates
  • Requesting an Appointment
  • Work/School Notes — New or a revision request.
  • Photos are not to be sent through email unless directed by your Physician to do so.
